Fill out the right passport application form; if you are using the DS-82 you will have to pay $72 to the US dept of state. And if you are using Passport form DS-5504, there is no question of paying any fee.
Fill in the application for name change on passport attached with the relevant documents, your old passport and two passport size photograph, put it into an envelop and mail it to the concerned passport office in your state.
You may need to wait for your passport to come to you by mail in 4 weeks’ time.
